ABSTRACT

RESUMO O hemangioma de coroide é um tumor vascular benigno, de coloração vermelho-alaranjada, bem delimitado, caracterizado por uma placa elevada. É um tumor raro, com prevalência de um caso a cada 40 tumores de coroide. O diagnóstico pode ser feito por meio da clínica associada à avaliação biomicroscópica e a exames complementares para diferenciação de outros tumores. O tratamento pode ser expectante nos casos assintomáticos. Para os casos sintomáticos ou com presença de fluido sub-retiniano, existem diversas terapias. O objetivo deste estudo foi relatar um caso de hemangioma circunscrito de coroide submetido a tratamento combinado de terapia fotodinâmica com verteporfina e injeção intravítrea de antiangiogênico (bevacizumabe). A decisão de tratar um hemangioma de coroide deve ser individualizada com base nos sintomas, na perda visual e em qualquer potencial de sua recuperação. O exame oftalmológico completo é necessário, mesmo em casos assintomáticos, para rastreamento precoce de doenças oculares.


ABSTRACT Choroid hemangioma is a benign, well-delimited orange-red, vascular tumor characterized by an elevated plaque. It is a rare tumor with a prevalence of one case in every 40 choroidal tumors. It can be diagnosed by the clinic associated with biomicroscopic evaluation and complementary tests to differentiate from other tumors. Treatment can be expectant in asymptomatic cases. For symptomatic cases or those with the presence of subretinal fluid, there are several therapies. The objective of this study was to report a case of circumscribed choroidal hemangioma submitted to combined treatment of photodynamic therapy with verteporfin and intravitreal injection of an antiangiogenic agent (bevacizumab). The decision to treat choroidal hemangioma must be individualized based on symptoms, visual loss, and any potential for recovery. A complete eye examination is necessary, even in asymptomatic cases, for early screening for eye diseases.

ABSTRACT

Resumo O hemangioma de coroide é um tumor benigno relativamente raro, que se apresenta de forma circunscrita ou difusa, sendo esta última normalmente associada à Síndrome de Sturge-Weber. Os tumores circunscritos manifestam-se de forma insidiosa, com o diagnóstico realizado comumente após o aparecimento de sintomas secundários. Apresentam como diagnóstico diferencial lesões graves e potencialmente letais, como melanoma de coroide e doença metastática. Neste relato descrevemos o caso de um hemangioma intraocular nodular avançado associado a descolamento hemorrágico da retina, evidenciando o desafio do diagnóstico diferencial devido às semelhanças clínicas e radiológicas compartilhadas pelos tumores.


Abstract Choroidal hemangioma is a fairly rare benign vascular tumor that can manifest in either circumscribed or diffuse type; the latter one is usually related to Sturge-Weber Syndrome. The circumscribed tumors have an insidious presentation and diagnosis is commonly made after the onset of secondary symptoms. Serious and potentially lethal lesions, such as choroidal melanoma and metastatic disease, may represent a differential diagnosis. In this report, we describe an advanced case of nodular hemangioma associated with hemorrhagic retinal detachment. This case highlights the challenge of differential diagnosis in intraocular tumors, due to their similar clinical and radiologic features.

ABSTRACT

ABSTRACT Uveal melanoma is the most common adult primary intraocular cancer. Although liver metastasis is common to the natural history of the disease, metastasis to the fellow eye is extremely rare. Here we report the case of a 59-year-old man with choroidal melanoma in his right eye who underwent enucleation at a different center. The patient was referred to our service 21 months postoperatively, complaining of decreased vision. He was found to have a new pigmented choroidal tumor in his left eye associated with liver disease. Ocular ultrasonography and liver biopsy with histopathological and immunohistochemical analysis were performed and confirmed the diagnosis. Few similar cases have been described in the literature. The differential diagnosis included primary bilateral choroidal melanoma and metastatic choroidal tumor from a primary skin melanoma.


RESUMO O melanoma uveal é o câncer intraocular primário mais frequente em adultos. Embora a metástase hepática seja comum à história natural da doença, a metástase para o outro olho é extremamente rara. Aqui relatamos o caso de um homem de 59 anos com melanoma de coroide em seu olho direito que foi submetido à enucleação em um centro diferente. O paciente foi encaminhado ao nosso serviço 21 meses após a cirurgia, com queixa de diminuição da visão. Foi encontrado um novo tumor de coróide pigmentado em seu olho esquerdo associado com doença hepática. Ultrassonografia ocular e biópsia hepática com exame histopatológico e imuno-histoquímico foram realizadas e confirmaram o diagnóstico. Poucos casos semelhantes foram descritos na literatura. O diagnóstico diferencial incluiu melanoma de coróide bilateral orimário e tumor coroidal metastático de um melanoma primário da pele.

ABSTRACT Purpose: To compare measurements of lesions clinically diagnosed as choroidal nevi using spectral domain optical coherence tomography (SD-OCT) and 10- and 20-MHz ultrasound (US). Methods: This prospective study, which was conducted between May 2011 and December 2011, evaluated eyes diagnosed with choroidal nevus via photographic documentation using 10- or 20-MHz A- and B-mode US (experienced examiner using both the transpalpebral technique and direct contact) or SD-OCT in the enhanced depth imaging mode (performed by a different examiner blinded to the US results). Anteroposterior (AP) and transverse (T) US sections corresponded to sections adjusted perpendicularly on SD-OCT. Results: We evaluated 14 eyes from 12 patients (six males, mean patient age= 64.5 years) diagnosed with choroidal nevus. The choroidal nevi of all samples had a melanocytic profile. Moreover, eight nevi were located at the equator, five nevi were located in the posterior pole (peripapillary in one sample), and one nevus shifted from the equator to the periphery. On SD-OCT, the maximum measurable dimension was 9 mm. The lesions in the posterior pole were easier to evaluate, and image acquisition of lesions located more peripherally was possible depending on patient cooperation. The accurate assessment of height was difficult. Baseline dimensions on 10- and 20-MHz US were larger than those determined via OCT. No significant differences in height were observed between US and SD-OCT. All parameters were statistically similar between 10- and 20-MHz US measurements. Conclusions: No significant difference in the AP and T diameters was observed between 10- and 20-MHz US measurements; however, these measurements (AP and T) were significantly higher than those obtained using OCT. No significant differences in height were observed among the techniques adopted.


RESUMO Objetivo: Comparar as medidas obtidas de lesões diagnosticadas clinicamente como nevus de coroide através da tomografia de coerência óptica de domínio espectral (Spectralis, Heidelberg Engineering, Inc.), ultrassonografia com 10 MHz e de 20 MHz. Métodos: Estudo prospectivo realizado entre maio e dezembro de 2011, avaliou olhos com diagnóstico de nevus de coroide, utilizando documentação fotográfica, ultrassonografia com transdutor 10-MHz e 20-MHz A- e B-mode e SD-OCT em modo de EDI, por um examinador diferente para cada técnica. Os cortes realizados perpendiculares entre si, correspondentes ao corte ântero-posterior e latero-lateral à ultrassonografia. Resultados: Foram avaliados 14 olhos de 12 pacientes (6 do sexo masculino), com média de idade média de 64,5 anos. Todos os nevus tinham um perfil melanocítico. Observou-se 8 nevus no equador, 5 no polo posterior (peripapilar em uma amostra), e 1 deslocado a partir do equador para a periferia. Em SD-OCT, a dimensão máxima mensurável foi de 9 mm. As lesões no polo posterior eram mais fáceis de avaliar e aquisição de imagens de lesões mais periféricas era possível, dependendo da colaboração do paciente. A avaliação precisa da altura era difícil. As dimensões usando transdutor 10-MHz e 20-MHz US foram maiores que as encontradas pelo SD-OCT. Não foram observadas diferenças significativas na altura entre métodos SD-OCT e US. Todas as medidas foram estatisticamente semelhantes entre 20-MHz e 10-MHz. Conclusão: Para o parâmetro AP e T não foi detectada diferença entre as medidas utilizando US de 10-MHz e de 20-MHz. Porém estas medidas se mostraram significativamente maiores em relação à medida obtida com OCT. Para a altura, não foram detectadas diferenças estatística em relação à técnica utilizada, US 10-MHz e 20-MHz e SD-OCT.

ABSTRACT

ABSTRACT We report enhanced depth imaging optical coherence tomography (EDI-OCT) features based on clinical and imaging data from two newly diagnosed cases of choroidal osteoma presenting with recent visual loss secondary to choroidal neovascular membranes. The features described in the two cases, compression of the choriocapillaris and disorganization of the medium and large vessel layers, are consistent with those of previous reports. We noticed a sponge-like pattern previously reported, but it was subtle. Both lesions had multiple intralesional layers and a typical intrinsic transparency with visibility of the sclerochoroidal junction.


RESUMO Relatamos as características na tomografia computadorizada óptica (EDI-OCT) de 2 pacientes recém diagnosticados com osteoma de coroide apresentando perda visual secundária à membranas neovasculares coroideanas. As características descritas em nossos 2 casos foram consistentes com trabalhos anteriores, exibindo a compressão da coriocapilar e desorganização das camadas médias e de grandes vasos. Notamos também o padrão em esponja anteriormente descrito, porém de forma discreta. Ambas as lesões tinham várias camadas intralesionais e uma transparência intrínseca típica com visibilidade da junção da esclero-coroideana.

ABSTRACT

ABSTRACT A 68-year-old man diagnosed with choroidal melanoma (CM) in the right eye underwent treatment with episcleral brachytherapy (I125) and transpupillary thermotherapy. Ultrasound, computed tomography, and magnetic resonance imaging were performed and revealed ocular recurrence of CM. Treatment with extended enucleation was performed. Macroscopic and microscopic examinations revealed extraocular extension and malignant cells, respectively. Immunohistochemistry demonstrated tumoral Melan-A and HMB-45 expression. No cytogenic abnormalities were detected with fluorescence in situhybridization of tumor cells using probes against chromosomes 3q27 and 8q24. The patient underwent adjuvant external beam radiotherapy for treatment of residual tumor tissue. This case represents the first reported case of recurrent CM with no cytogenetic abnormalities and the absence of metastatic disease, despite a number of the poorest prognostic factors.


RESUMO Um homem de 68 anos de idade com diagnóstico de melanoma de coroide no olho direito foi submetido a tratamento com braquiterapia episcleral (I125) e termoterapia transpupilar. Ultrassonografia, tomografia computadorizada e ressonância magnética foram realizadas para avaliar a presença de recorrência ocular ou doença sistêmica. Enucleação ampliada foi realizada para tratar a recorrência ocular. O exame macroscópico e microscópico revelou o tipo de célula tumoral e a extensão extraocular. Colorações por Melan-A e HMB-45 foram realizadas. A fluorescência por hibridização in situ com sondas para os cromossomos 3q27 e 8q24 não mostraram anormalidades citogenéticas. O paciente foi submetido a radioterapia externa adjuvante para o tratamento de tumor residual orbitário. Este caso representa a o primeiro relato de paciente sem anomalias citogenéticas e sem doença metastática, apesar de demonstrar alguns dos mais pobres fatores prognósticos.

ABSTRACT

The authors report five cases of uveal tumors managed with internal eye wall resection and evaluate the long-term results. Patient age varied from 31 to 61 years. Four patients were female and one was male. Uveal tumors involved ciliary body and choroid in three patients and choroid alone in the other two. Histopathological examination demonstrated three uveal melanomas, one leiomyoma, and one meduloepithelioma. The largest tumor diameter ranged from 8 to 15 mm and tumor thickness from 5 to 11 mm. Follow-up of the patients was over a period of 6 to 23 years. Final visual acuity was 20/20 in 3 patients and 20/200 in one patient, and in the fifth patient, the affected eye was enucleated. No cases of metastatic disease or local recurrence of the tumor were observed, suggesting that internal eye wall resection is a valid alternative for the treatment of selected cases of uveal tumors.


Os autores relatam 5 casos que apresentavam tumores uveais e foram submetidos à remoção cirúrgica em bloco do tumor, avaliando resultados de acuidade visual, índice de retenção do globo ocular e de doença metastática. Quatro mulheres e um homem, com idade entre 31 e 61anos foram submetidos ao procedimento. O tumor envolvia corpo ciliar e coroide em três olhos e somente a coroide em dois. A histopatologia demonstrou que 3 tumores eram melanomas, um leiomioma e um meduloepitelioma. O maior diâmetro dos tumores foi entre 8 e 15 mm e a espessura entre 5 e 11 mm. O tempo de seguimento dos pacientes foi de 6 a 23 anos. A acuidade visual final foi de 20/20 em 3 olhos, 20/200 em outro e um olho foi enucleado. Não foram observados casos de doença metastática ou recorrência tumoral, sugerindo que a ressecção em bloco de tumores uveais é uma alternativa válida para o tratamento destes tumores em casos selecionados.

ABSTRACT

Metastasis in choroidal melanoma is frequent on advanced diseases, involving mainly the liver, lungs and central nervous system. We report a case of cutaneous metastasis in choroidal melanoma because is an unusual condition, even in advanced disease.


A ocorrência de metástases do melanoma de coróide é frequente na doença avançada, acometendo principalmente fígado, pulmões e sistema nervoso central. Relatamos um caso de melanoma de coróide com metástases cutâneas, por se tratar de acometimento raro, mesmo em casos avançados da doença.

ABSTRACT

Melanomas oculares correspondem a 5 por cento de todos os melanomas e 85 por cento deles têm origem no trato uveal. Melanoma uveal é o tumor maligno intraocular primário mais comum no adulto. Relatamos neste artigo um caso de melanoma uveal em paciente, sexo feminino, 31 anos, com quadro de fotopsia, hiperemia e baixa da acuidade visual no olho esquerdo com evolução de quatro meses. Apresentava ao exame oftalmológico acuidade visual menor que 20/400, grande massa tumoral na região nasal retroiriana, com deslocamento anterior do cristalino, estreitamento da câmara anterior e descolamento seroso da retina. A ecografia sugeriu tratar-se de grande massa tumoral suspeita de melanoma de coróide com invasão do corpo ciliar. A confirmação diagnóstica foi possível por meio do exame anatomopatológico.


Ocular melanomas correspond to 5 percent of all melanomas and 85 percent of them have its origin in the uveal tract. Uveal melanoma is the most commom primary intraocular malignant tumor in the adult. In this article, a case of uveal melanoma in a 31 year-old female patient, with photopsia, hyperemia and low visual acuity in the left eye with evolution of 4 months is presented. In the ophthalmologic examination, visual acuity was lower than 20/400, a large tumoral mass was noted at the nasal region behind the iris with anterior lens displacement, anterior chamber narrowing and serous retinal detachment. The ocular echography suggested a large tumoral mass as a choroidal melanoma extending to the ciliary body. The confirmation diagnosis was possible through the histopathologic examination.

ABSTRACT

La metástasis coroidea es una entidad poco frecuente y está asociada usualmente a un mal pronóstico, siendo el cáncer de mama su principal causa. Se presenta el caso de un paciente femenino de 47 años de edad con antecedente de cáncer de mama estadio IIIA diagnosticado y tratado en 1998, permaneciendo libre de enfermedad locorregional y a distancia hasta julio de 2005 cuando presenta disminución de la agudeza visual basados en los test clínicos se le diagnosticó metástasis coroidea en ojo derecho. Inicia tratamiento local y sistémico con evolución satisfactoria y remisión completa de la lesión corroborada por estudios complementarios.


The choroid metastasis, are not frequent in intraocular malignant tumors, the breast cancer, is the primary tumor more frequently responsible for ocular metastases. These reports describe a feminine 47 years old diagnosed with infiltrating ductal carcinoma of her right breast classified how state III. A treated in 1998, she was free of disease until July of 2005, she complained of diminished visual acuity of her right eye. Based on the clinical and test outcomes the following diagnosis was reached: Choroid metastasis in the right eye, from a known primary tumor. The patient started treatment local and systemically chemotherapy with excellent response.

ABSTRACT

Intraocular metastatic tumor is the commonest intraocular malignancy in adults, with uveal tract the commonest site. In nearly 85% of cases the choroid is the afflicted site due to its vascularity. Breast and lung are the common primaries. In breast primaries, this could be the first metastatic disease. This condition should always be kept in mind in patients with visual symptoms. Fundus examination, ultrasonography and CT/MRI of the orbit help in diagnosis. Early recognition and timely treatment can save the visual function thus imparting good quality of life to the patient. External beam radiotherapy is a good local form of treatment. Hormone therapy in hormone receptor positive tumors can have additional benefits. Here we report a case of unilateral solitary choroid metastasis in a case of breast cancer treated with external beam radiotherapy.

ABSTRACT

A raíz de un caso de osteoma de coroides que se diagnóstico en el Instituto Barraquer de Barcelona se hizo una revisión bibliográfica del tema y se revisaron las fichas con el diagnóstico de osteoma de coroides entre los años 1982 y 2004, encontrándose 29 casos diagnosticados como tal. La revisión de las fichas descartó a 13 pacientes que no cumplían con los parámetros para osteomas, ya sea por mala interpretación de la ecografía o de otros exámenes. Los 16 restantes, 9 habían concurrido a una segunda opinión ya con el diagnóstico de osteoma. Los exámenes hechos en nuestro centro corroboraron el diagnóstico. Cinco pacientes nos consultaron por primera vez por disminución de la agudeza visual y en nuestro examen se encontró el osteoma. Otro caso nos consultó por jaqueca y diagnóstico previo en otro centro de coroiditis posterior. Un caso fue un hallazgo casual en una ecografía practicada por nosotros.

ABSTRACT

Four cases of malignant melanoma of the choroid are reported due to rarity of the condition in India. One of the cases presented with Naevus of Ota. All the cases had typical clinical and investigative features. All cases were enucleated. Histopathologically three of them were of mixed type and one was of the epithelioid type. Two of the cases were seen in patients below 40 years of age.

ABSTRACT

Apresenta-se o caso de uma paciente de cor branca, com 31 anos de idade portadora de melanoma coróideo de tipo fasciculado. Dois anos e quatro meses mais tarde, após a enucleaçäo apareceu mancha anegrada, no ponto lacrimal do mesmo lado. Quatro anos depois ela morreu por processo metastático. Previamente foi feita biópsia hepática e o exame revelou a mesma estrutura fasciculada. Estuda-se a correlaçäo entre melanoma e "ativaçäo" névica, pensa-se que as células do melanoma fasciculado constituem um clone distinto e como o termo fasciculado é impreciso, faz-se a proposiçäo de melanoma peritelial para os melanomas com estrutura fasciculada. Afirma-se que os tumores melanocíticos säo pleoclonais em sua grande maioria e, por este motivo, o diagnóstico de um tipo especial oferece dificuldades. A expresäo melanoma peritelial com áreas epitelióides pleomórficas é mais correta para o caso apresentado
